POSITION DESCRIPTION
This position partners with the Lumity team to assess and address program participants’ social emotional and support service needs to obtain and retain employment and provide ongoing support with program alumni. This position will work collaboratively with their peers to ensure each youth has an individualized plan that addresses social-emotional, employment and life skills to successfully launch and maintain their career. This position works closely with a LCSW who contracts with Lumity and this position reports to the Chief Program Officer.
PRIMARY RESPONSIBILITIES
- Build relationships with youth to assess their social emotional and support services that may impact their employment and life success (e.g., transportation, day care, housing, health care, work clothes and tools). Connect youth to appropriate resources to remove barriers.
- Create an individualized plan for each youth using the Casey Life Skills Assessment (or other assessments) to prepare youth for life skills in daily living; relationships and communications; housing and money management; self-care; work and study life; and permanent connections.
- Determine the trauma informed approach to ensure youth are successfully equipped with life skills to enter long-term, full-time employment or an accredited postsecondary credentialed program.
- Assist the Program Facilitators in enrollment and administration of program curriculum.
- Facilitate “opening circles” and restorative conversations with program participants.
- Gather and report data for grant and program report requirements.
- Establish and maintain relationships with community assistance programs and meet with them regularly to better understand the services they provide and how those can benefit Lumity participants.
- Work collaboratively with social work interns from local colleges/universities and facilitate case management review sessions.
- Represent Lumity at meetings, conferences, and events and conduct presentations to various audiences.

Requirements
Trauma-Informed Social Service/Social Work Professional with Experience Working with Youth of Color
Candidates with an MSW, Psychologist or relevant professional social service degree is preferred.
Minimum of 3-years of experience providing social emotional and support services with young adults who have experienced higher levels of trauma.
- Technology Skills- Advanced level proficiency with Microsoft Office Suite and remote platforms such as Google Meet and Zoom; Ability to learn and adapt to new technologies quickly.
- Detail/Accuracy- Demonstrate accuracy and thoroughness; Monitor own work to ensure quality, organized and timely execution; Maintain strong attention to detail.
- Communication- Clearly and persuasively communicate by listening and seeking clarification; Ability to lead meetings; Demonstrate excellent written and verbal communication in all activities.
- Initiative/Teamwork- Demonstrate ability to work independently and as part of a team; Exhibit strong critical thinking skills and sound judgment, including prioritization and time management skills; Work collaboratively in a multicultural, team-based work environment.
- Professionalism- Maintain professional appearance and demeanor; Prioritize internal and external customer interactions and respond appropriately; Treat others with respect and consideration; Accept responsibility for own actions; Embody a growth mindset.

Work Schedule/Location
Full-time, 37.5 hours/5 days a week Monday-Friday with some evening and occasional weekend work. This position is a combination of remote and onsite work based on workload. Applicants will need to be able to commute/transit to work at Lumity’s main office located at 1132 S. Wabash Ave. in the South Loop, at 1010 Madison in Maywood, and possibly other locations in the Chicagoland area.
