Job Summary
The Structural Heart, RN Coordinator serves as a primary clinical navigator for patients undergoing evaluation and treatment for structural heart disease, with a primary focus on Transcatheter Aortic Valve Replacement (TAVR).
Based in Plano, this role coordinates care across the patient's Structural Heart journey. The RN Coordinator works closely with interventional cardiologists, cardiac surgeons, advanced practice providers, imaging teams, procedural staff, referring providers, and administrative teams to ensure patients progress safely and efficiently through a complex multidisciplinary care pathway.
This position also supports the program's outreach strategy, currently including Structural Heart clinic coverage on the BSW Rockwall campus, approximately twice monthly. The coordinator will help develop referral relationships, support clinic growth, and establish a stronger Structural Heart presence within the community as outreach volume expands.
Essential Functions of the Role
• Serve as a clinical point of contact and navigator for TAVR patients throughout evaluation, treatment, and follow-up.
• Review incoming clinical records to identify patient needs and facilitate appropriate next steps.
• Coordinate Structural Heart diagnostic testing, including echocardiography, CT imaging, cardiac catheterization, laboratory testing, and other studies as indicated.
• Prepare patients for multidisciplinary Structural Heart/Heart Team evaluation.
• Coordinate consultations with interventional cardiologists, cardiac surgeons, APPs, and other specialists.
• Provide patient and family education regarding the Structural Heart evaluation, TAVR procedure, preparation, recovery, and follow-up expectations.
• Identify clinical or operational barriers that may delay care and work collaboratively with the multidisciplinary team to resolve them.
• Support procedure readiness, including review of required testing, documentation, education, and pre-procedure requirements.
• Participate in post-procedure patient navigation and ensure required follow-up is completed, including 30-day and one-year visits.
• Support completion of required Structural Heart quality and registry elements, including patient assessments and follow-up requirements.
• Maintain clear communication with patients, families, referring providers, and members of the Structural Heart team.
• Provide onsite RN Coordinator support for Lake Pointe/Rockwall Structural Heart outreach clinics approximately twice monthly.
• Partner with physicians and program leadership to grow outreach volume and strengthen referral relationships within the Lake Pointe/Rockwall market.
• Assist with development and refinement of workflows as outreach services and patient volumes expand.
• Maintain flexibility to support evolving Structural Heart program needs across clinic and outreach settings.

Qualifications
• Grad of an Accredited Program
• Specialized field of study should be nursing.
• RN license.
• Must have BLS certification or get it within 30 days of starting in this role.
